Get ready for a lot of acronyms: Between 2015 – 2019, the Patient Centered Primary Care Collaborative (PCPCC) implemented a project called Transforming Clinical Practices Initiative (TCPI) thanks to funding from the Centers for Medicare and Medicaid Services (CMS)

Still with us?

Throughout the program, PCPCC and their partners created a full suite of customizable tools and resources for implementing patient and family engagement strategies to transform primary care practices. With the program beginning to wind down, PCPCC needed to ensure this work was promoted and sustained beyond the grant cycle.
